Kentucky State Board Permit Information
- When taking our Eyelash Extension Training, you will receive a Kentucky State Board Lash Permit / License.
- Having a Kentucky State Board Lash Permit / License will allow you to run your own business, work in a salon, etc.

Program For Under 18 Years Of Age
We are making this program available to our young lash technicians to train with our academy. We would offer a full training course to students under 18 years of age with parental consent and supervision. However, due to the laws and regulations by the Kentucky Stateboard we wouldn't be able to offer a legal certificate until the age of 18. Signing and returning this form will guarantee that upon turning 18 the certificate will be extended to the trainee per completion of the course and the student may practice and can’t perform inside a salon until of age. Students are eligible to also sit in on a refresher course once the certificate is granted to be salon ready.
